The gold standard to determine airway hyperresponsiveness, used to characterize the likelihood of asthma, remains the bronchial provocation test using methacholine. The long awaited ERS/ATS guidelines published in 2017, diverge from the former 1999 guidelines and make explicit recommendations around such topics as: nebulizer characteristics, mode of delivery of methacholine, concentration of methacholine for dosing schedule, provocative dose, and characterizing severity of AHR, are among only some of the updates.
Dr. Frans de Jongh will review the details of the new guidelines, as well as contrasting the changes from former standards where such things as PC20 vs PD20 are concerned, and recommendations to coach the maneuver as tidal breathing versus inhalation to TLC and why these maneuvers should be avoided.
